Understanding Sciatica
Sciatica describes compression or irritation of the sciatic nerve — or its lumbar nerve roots from L4 through S5 — producing radiating pain that travels from the lower back through the buttock and down one leg. It is typically unilateral. Up to 40% of adults experience sciatica at some point in their lives, with prevalence rising after 50 due to disc degeneration, spinal stenosis, and piriformis syndrome.
The Inflammation Connection
Most sciatica involves both mechanical compression and local inflammation around the affected nerve root. The inflammatory cascade — prostaglandins, cytokines, leukotrienes — amplifies pain signals independent of the structural compression itself. This is why anti-inflammatory interventions can produce meaningful symptom relief even without addressing the mechanical cause directly.
Movement: Counterintuitive but Supported
Bed rest worsens outcomes in most sciatica cases. Walking — even short distances — maintains circulation to nerve tissue, reduces inflammatory mediators, and prevents the deconditioning that prolongs episodes. Targeted stretches (piriformis stretch, knee-to-chest, supine hamstring stretch) have evidence for symptom reduction. The McKenzie method, which uses specific extension-biased exercises, has strong evidence for disc-related sciatica in particular.
Anti-Inflammatory Nutritional Approaches
Omega-3 fatty acids (EPA/DHA) reduce prostaglandin E2 and leukotriene B4 production. Studies show reduced inflammatory markers and pain scores at 2-3g combined EPA+DHA daily.
Curcumin/turmeric inhibits NF-κB, the master inflammatory regulator, with effects comparable to some NSAIDs in inflammatory marker studies. Bioavailability is the main challenge — piperine (black pepper extract) increases curcumin absorption roughly 20-fold.
Magnesium is involved in nerve conduction and muscle relaxation; deficiency increases pain sensitivity. Glycinate or malate forms are preferred for nerve applications.
Alpha lipoic acid is an antioxidant with specific affinity for nerve tissue and supporting evidence for neuropathic pain reduction.
B vitamin complex (B1, B6, B12) supports myelin sheath integrity. B12 deficiency in particular amplifies neuropathic pain signals.
What Requires Medical Evaluation
New onset sciatica should be medically evaluated. Red flags requiring urgent care include bowel or bladder dysfunction, bilateral symptoms, saddle anesthesia (numbness in the groin or inner thighs), and progressive neurological weakness. Most sciatica resolves within 12 weeks with conservative management, but those red-flag presentations can indicate cauda equina syndrome or other emergencies.
Complementary Approaches With Evidence
Spinal manipulation (chiropractic or osteopathic) shows moderate evidence for short-term relief. Acupuncture has modest evidence for pain reduction. Cold/heat alternation provides symptomatic relief through pain gate mechanisms — useful symptom management but not a structural intervention.
